Best acetyl-L-carnitine supplement
James BevanUpdated Nov 2, 2026
Acetyl-L-carnitine, or ALCAR, is a form of carnitine that turns up in the bigger brain formulas, usually at 500 to 1,000 mg. But its trials gave 1,500 to 3,000 mg a day for months, nearly all to older adults with memory problems. So we've checked every ALCAR capsule and stack we've audited against those trials, worked out what each costs a day, and picked the one we'd buy if you've decided to try it.

What our audit found
We checked every acetyl-L-carnitine capsule and stack we've audited against the trials, and these are the five things that matter most if you're choosing one.
-
The trials behind it are in people with memory problems
The 1,500 mg a day we check labels against comes from a 2018 trial in 56 adults with dementia after strokes, who were already taking donepezil.1 The larger body of trials is in mild cognitive impairment and Alzheimer's disease, and a Cochrane review of L-carnitine trials in healthy people couldn't draw any conclusions.25
-
No stack we audited reaches 1,500 mg
The eight brain supplements with ALCAR print 100 to 1,000 mg a serving, and the two we've reviewed give 500 mg and 800 mg. So every stack sits below even the patient trials.
-
Three singles' directions reach 1,500 mg
Thorne's label says one 500 mg capsule two to three times a day, and NaturaLife's says two capsules, 1,500 mg.1113 NOW's directions get there too, at three capsules, and Nutricost's 1,500 mg bottle prints 1,500 mg as a three-capsule serving. Double Wood's label stops at 1,000 mg a day.141516
-
Only Thorne's amount is checked from outside
NSF lists Thorne's capsule under its NSF/ANSI 173 standard, which checks that the label's ingredients and amounts are in the bottle.12 Double Wood publishes an independent lot report that confirms identity, and the others publish nothing.16
-
The biggest trial found nothing
In a 1996 trial, 431 people with Alzheimer's took 3,000 mg a day or a placebo for a year, and both groups declined at the same rate.4 The smaller trials and the meta-analysis that pooled them found small gains.2
Best acetyl-L-carnitine supplements
Thorne's capsule holds 500 mg, and its label says one two to three times a day, so three give 1,500 mg for $1.60 a day. It's the one we'd choose, because NSF checks that the amount on the label is in the bottle. NaturaLife's two capsules give the same 1,500 mg for less, but it publishes no lab report.
500 mg of ALCAR a capsule, and the label says one two to three times a day, so three give 1,500 mg for $1.60. Listed under NSF/ANSI 173, which checks the label's amounts.

We also audited four other ALCAR capsules. NOW's 500 mg capsule scores 8.0 out of 10, and its label says one or two capsules once or twice a day, so three reach 1,500 mg.14 Nutricost's 1,500 mg bottle scores 7.9, three capsules a serving.15 Neither publishes a lab report. Double Wood scores 6.6, because its label stops at two 500 mg capsules a day, though it comes with an independent lot report.16 Nutricost's 500 mg bottle, whose label says one capsule a day, scores 3.9.
Acetyl-L-carnitine in brain supplements
The table ranks the brain supplements we've audited that contain ALCAR by audit rating, with the amount each one prints and what the formula costs for 1,500 mg of it.

The cost column scales each formula's full daily price to 1,500 mg of ALCAR, without rounding to whole capsules and more than any serving holds, so it's a price comparison and not a dose to take.
If ALCAR is why you're buying, two NaturaLife capsules give 1,500 mg for 34 cents a day.13 That's more than any stack here, at a fraction of the price.
Is acetyl-L-carnitine worth buying, and who it suits
We wouldn't buy acetyl-L-carnitine as a focus supplement. Its trials are in older adults with memory problems, given 1,500 to 3,000 mg a day for months, and the gains were small, while there's almost nothing in healthy people.235
A 2003 meta-analysis of trials in mild cognitive impairment and mild Alzheimer's found ALCAR did a little better than placebo, with the gap showing by three months and growing after that.2 A Cochrane review the same year looked at 11 Alzheimer's trials. It found more people rated as improved at 12 and 24 weeks, but no benefit on thinking tests, and it concluded there was no evidence to recommend routine use.3 The trial we benchmark against, Yang 2018, found its people held steady on a memory and thinking test while the placebo group slipped, but none of its other measures differed.1
So it doesn't suit you as a daily focus capsule if your memory is fine. Cochrane found only two small trials of L-carnitine in healthy young adults. The one with data found no effect after three days, and the authors said there was too little to draw any conclusion.5 If you're worried about your memory, see a doctor first, because the trials were in people with a diagnosis. FitnessVolt's October 2026 ranking makes the same point about healthy adults, though it tells you to start with a 500 mg capsule, a third of the trial dose.17
The NIH Office of Dietary Supplements says carnitine can cause seizures in people with a seizure disorder, and that anticonvulsants such as valproic acid lower carnitine levels.9 Drugs.com says to check with your doctor first if you have heart or blood vessel disease, kidney problems, diabetes or seizures.10 If you take a prescription medicine, ask your doctor or pharmacist before you try it.
How much to take, and when
Here's what the trials gave, and how it maps onto capsules.
- Study dose benchmark
- 1,500 mg a day
- Doses in the patient trials
- 1,500 to 3,000 mg a day
- Time in the trials
- Usually 3 months or longer
The trials gave 1,500 to 3,000 mg a day, usually split into three doses, and the 2003 meta-analysis pooled trials of three months or longer.124 We check labels against 1,500 mg, the 500 mg three times a day that Yang 2018 used for 28 weeks.1
Thorne's capsule holds 500 mg, and its label says one capsule two to three times a day, so it matches the trial schedule exactly.11 NaturaLife's serving is two capsules, 1,500 mg, taken once a day.13 Neither label goes past 1,500 mg, and we wouldn't take more than a label recommends. Body Science Review's guide, updated in July 2026, suggests 500 to 1,000 mg a day for focus, which is below anything the trials tested.18
What the trials found
Nearly all of ALCAR's trials are in older adults with memory problems, and they run for months.
ModerateSmall gains in mild cognitive impairment and early Alzheimer's at 1,500 to 3,000 mg a day for months, and none in the largest trial. Almost no trials in healthy adults.
In people with a diagnosis, the smaller trials found small gains, and the largest found none. In older adults without dementia, trials have reported less fatigue and better thinking scores, but they all come from one research group in Catania, Italy.12467

ALCAR does get into the brain. In 11 people with Alzheimer's, levels in the fluid around the brain rose in step with blood levels on 1,500 mg a day by mouth.8 We found no trial comparing it with plain L-carnitine for thinking.
What it's stacked with
ALCAR turns up in the broad brain formulas, usually next to phosphatidylserine, tyrosine or citicoline. These are its usual neighbors, and where we found them.
- PhosphatidylserineModerateNext to ALCAR in six of the eight stacks we audited, including Qualia Mind and Hunter Focus.
- L-TyrosineModerateNext to ALCAR in six of the eight stacks we audited, including Qualia Mind and Hunter Focus.
- CiticolinePreliminaryNext to ALCAR in five of the eight stacks we audited, including Qualia Mind and Hunter Focus.
